Care management is personalized support that helps individuals and families navigate healthcare and daily living needs. A care manager works alongside you to coordinate services, advocate for your best interests, simplify communication between providers, and make sure your care plan reflects your goals and values.

I offer a range of services, including: care coordination and planning, medical appointment accompaniment, resource identification and advocacy, transitions of care (home to facility, hospital to rehab, etc.), and personalized care plans.

My goal is to connect you with the right care and services to support your needs.

I offer several service packages and payment plans to fit different needs and budgets. Prices vary depending on the level of support and time required.

Most care management and advocacy services are not covered by Medicare or private insurance. My Way Care does not currently accept insurance. We can discuss your options during our consultation and determine the best way to move forward!

I offer both! Depending on the clients needs and location. In-person visits are ideal for medical appointments and facility check-ins, while virtual meetings can help with planning, updates, and emotional support.

I currently provide services in Palm Beach County, but I'm open to discussing virtual options or expanding services in the future. Reach out to see if I can accommodate your location.
